For decades, the message was clear and unambiguous; constantly emphasized in public health campaigns and repeated by doctors at the end of examinations: Complete your antibiotic treatment, even if you feel better. Stopping too early risked leading to the development of resistant bacteria. This dogma is now being seriously questioned by science. And the change is dramatic: The emerging new idea suggests that completing antibiotic treatment may, conversely, increase the risk of antibiotic resistance.

This idea arose in the early days of antibiotic development in the 1940s and persisted because it was "simple and easy to implement." Simple, yes. But without solid evidence, it is far less valid. A 2017 opinion piece published in the BMJ, signed by infectious disease specialist Professor Martin Llewelyn and nine other clinicians, addressed the issue directly: "This idea is not supported by any evidence and using antibiotics for longer than necessary increases the risk of resistance."

In fact, upon reflection, the mechanism is quite logical. Antibiotic resistance can develop through repeated exposure of bacteria to antibiotics: they adapt to survive subsequent exposures and gradually render the antibiotics ineffective against infections they were previously able to eliminate. The longer the exposure time, the stronger the selective pressure. Therefore, continuing to take the pill after the infection has clinically cleared subjects your gut microbiota to unnecessary bombardment, leading to the emergence of the most resistant bacteria more strongly.
